At your first appointment you will be given a Health Questionnaire to complete, as well as paperwork that includes information such as your address, emergency contacts, and insurance information. Please arrive 20 minutes before your appointment to complete this paperwork before being seen by your surgeon. If you would like, we will mail the forms for you to complete before your visit.

Please be sure to bring a complete list of all of your medications, including doses, with you to your appointment. Remember to include all herbal supplements, as well. Medication errors can be serious and you can help us to avoid these by keeping us up to date.
Payment and insurance issues:
If we are contracted with your insurance company we will bill your insurance company. As part of these contracts we are required to collect copays and these can be handled at your visit by cash, check, or major credit card.
**If you do not have insurance, please note that we will collect $280.00
up front before your consultation. Please keep in mind, that this amount is an estimate and final charges/fees may be billed to you.**
What to bring with you to your first appointment:
- All New Patient Paperwork listed above.
- Driver License or Photo ID.
- Insurance Cards.
- Medication list.
- Remember to bring any additional paperwork with you that might pertain to your visit, including outside records.
- A list of questions – this helps to avoid that tendency we all have to forget to ask questions when we are at the doctor’s office.
